For patients with early and intermediate Dry Age-Related Macular Degeneration (AMD), Photobiomodulation (PBM) therapy is an emerging, non-invasive treatment that may help support retinal function and slow disease progression.

While no treatment can cure Dry AMD, PBM is a low-risk therapy that has shown potential in improving contrast sensitivity, retinal metabolism, and reducing oxidative stress all of which may contribute to better long-term visual outcomes.

To ensure PBM is appropriate and beneficial for each patient, we integrate it into our Dry AMD Screening Service, using state-of-the-art imaging and functional tests to establish a baseline before starting treatment and to objectively measure response after each cycle of therapy.

What is Photobiomodulation (PBM)?

PBM, also known as Low-Level Light Therapy (LLLT), is a pioneering non-invasive treatment that uses specific wavelengths of red and near-infrared light to stimulate cellular activity, enhance energy production, and reduce inflammation within retinal cells. By targeting the mitochondria the energy centres of the cells. PBM may help support the function of retinal cells and potentially slow down the degenerative processes associated with Dry AMD.

Who Can Benefit from PBM?

PBM is currently being explored as a treatment for patients with early and intermediate Dry AMD. Studies suggest that PBM may help improve retinal metabolism and visual function in some patients. However, results vary, and ongoing monitoring is essential to assess effectiveness. How Does PBM Work?

PBM delivers targeted wavelengths of red and near-infrared light to the retina, where it is absorbed by mitochondria within the retinal cells. This process is believed to:

  • Support Cellular Energy Production (ATP Synthesis). May help retinal cells function more efficiently.
  • Reduce Oxidative Stress. A key factor in AMD progression, oxidative stress can damage retinal pigment epithelium (RPE) and photoreceptors. PBM may help neutralise some of this damage.
  • Minimise Inflammation. Chronic inflammation plays a role in Dry AMD progression, and PBM has been suggested to reduce inflammatory responses in retinal tissue.
  • Support Cell Function & Repair. PBM may promote retinal cell health and delay degeneration in some patients.

PBM is painless, non-invasive, and performed in a comfortable outpatient setting.

Monitoring PBM Effectiveness

At The Retina Clinic London, we do not offer PBM without carefully tracking its effects on each patient.

Baseline Assessment Before Starting PBM. We conduct comprehensive imaging and vision testing before treatment, including:

  • High-Resolution Macula OCT. To assess retinal structure and photoreceptor integrity.
  • Ultrawidefield Imaging & OCT-A. To check for any vascular changes.
  • Electroretinography (ERG). To evaluate retinal function.
  • Microperimetry. To map and monitor central visual sensitivity.
  • Visual Acuity & Contrast Sensitivity Testing. To measure any functional improvements.

Ongoing Monitoring After Each Treatment Cycle. PBM is delivered in cycles of 9 treatment sessions. After each cycle, we repeat key diagnostic tests to determine if the therapy is beneficial for the patient.

Tailored Treatment Plans. Based on objective results, we advise whether PBM should continue, be adjusted, or whether other treatment pathways should be considered.

If a patient progresses to late-stage Geographic Atrophy (GA) or converts to Wet AMD, we adjust the treatment plan accordingly, offering:

Syfovre (Pegcetacoplan) Injections for GA. A therapy that has been shown to preserve up to 30% of vision over several years.

Anti-VEGF Injections for Wet AMD. To halt abnormal blood vessel growth and prevent rapid vision loss.

Clinical Research on PBM for Dry AMD

PBM is still being investigated, but some clinical trials suggest potential benefits for patients with Dry AMD.

LIGHTSITE I & II Clinical Trials. Found that PBM treatment may lead to:

  • Modest improvements in Best Corrected Visual Acuity (BCVA). Some patients gained 5 ETDRS letters.
  • Enhanced Contrast Sensitivity. Better ability to distinguish details in low-light conditions.
  • Reduction in Drusen Volume. In some cases, PBM appeared to reduce early structural damage associated with Dry AMD.
  • Long-Term Potential. Improvements were maintained across multiple treatment cycles in some patients.

However, not all patients respond to PBM, which is why ongoing assessment and personalised monitoring are key to determining its suitability.
